Accreditation Supporting Documentation (FY26)

Libraries filing an application for accreditation and Direct State Aid Tier Level must submit the following documentation for review.
Documentation is broken down by tier and listed below. Mark which documents you are submitting. If you mark a document then another box will appear asking you to attach the document. You will only be asked to attach the documents you mark.
