Complete the following to request approval to retake either the NAPLEX or MPJE, Iowa Edition examination. Only one examination may be requested per application. If you would like to request approval to retake both exams, a separate application, fee, and NABP registration is required. To request approval to retake the NAPLEX or MPJE, Iowa Edition:
Submit this application, including uploading of your study plan if this is your third or greater attempt of the same examination.
Submit a nonrefundable fee of $36 (payable to the Iowa Board of Pharmacy) to the Iowa Board of Pharmacy, 6200 Park Ave., Ste. 100 Des Moines, IA 50321.
Complete the Internet-based NAPLEX / MPJE registration form and credit card payment fee process, which can be found at nabp.pharmacy.com.
