Media Request

This form is for members of the news media (television, newspaper, blog reporters). If you are not a member of the news media but need to contact DIAL about a specific reason, please use the regular Contact Form that will route your request to the appropriate staff.

Reports

  • Health Facilities Reports. Any report that DIAL is legally allowed to release for an Iowa health facility (assisted living facilities, elder group homes, hospices, hospitals, intermediate care facilities, nursing homes, residential care facilities, etc.) are available via the Health Facilities Division database, accesible online. Reports are only available once finalized and posted on this site. Choose "Entity Search" from the menu. From there, you will be able to search by health facility name, type, or location to find reports. 
  • Food Establishment Reports. Routine inspection reports for restaurants, bars, grocery stores, convenience stores, etc., are available on the department's food inspection website. You can query by several fields. When the record you are searching for comes up, it will show the most recent routine inspection and the number of violations (if any). Click the notebook icon on the right side of the entry to view the report, and click "other inspections" to pull up other routine inspections and follow-up inspections for that business.

Administrative Hearings Records

If you are inquiring about records involving an administrative hearing handled by DIAL, please contact the agency involved in the proceeding.

DIAL is not the legal custodian of records generated as a result of a hearing (please see Iowa Code 10A.105(4)).

Records Requests

Your records request will normally be fulfilled within 10 business days. However, in some cases, these requests may take longer than usual. We will do our best to reply in a timely manner and will communicate with you about the reason for any possible delay. Thank you for your understanding.

If your request will take longer than three staff hours to fulfill, there may be a cost to you. If it is estimated that your request will incur a charge, we will communicate that to you prior to working on the request, including the estimated cost. We can work with you to try to narrow the scope of your request in order to eliminate the possibility of an incurred cost.

Records requests may also be made by phone, fax or mail, or in person.
